Client consent form

To help us in our endeavour to provide the best possible service for your pet and to ensure that we comply with The animal health & welfare act (2006), we keep a detailed client record system.

Below is a client details and consent form to be completed on your initial visit. Upon subsequent visits you will be given an opportunity to advise us of any changes.

Barking beauties have strict shop policies which we would request all customers read.

Further to this is a form you will be required to sign after each visit to the salon to acknowledge that:

• We have made known to you any adverse health or behavioural problems identified by us.
• You are happy with the trim we have used.
• You are happy with the condition you took your pet home in.
